Does the alkaline diet work, and does food pH affect your health?
No. Your blood pH is held steady by your kidneys and lungs no matter what you eat, so food can't make the body "acidic" or "alkaline" the way the diet claims. Eating more vegetables is good for you, just not because it changes your pH.

What the research says
The core idea of the alkaline diet is that "acidic" food (meat, grains, sugar) makes the body acidic and that "alkaline" food (vegetables, fruit) corrects it. The physiology says otherwise. Blood pH sits steady around 7.35–7.45 and is tightly regulated by the kidneys and lungs; what you eat does not move it. The kidneys do adjust urine pH, so peeing more acidic or more alkaline after a meal is the kidney doing its job, not a sign that the whole body has shifted.
The two health claims that sell the diet hardest are protection against osteoporosis and cancer, and both have been examined closely. The bone claim rests on the notion that an "acidic" diet leaches calcium from the skeleton. Fenton's meta-analysis of calcium-balance studies found that a higher acid load does raise calcium in the urine, but does not produce a net loss of calcium from the body, and that there is no evidence the diet drives bone loss. A later systematic review that tested the link against formal criteria for causation reached the same conclusion: no causal association between dietary acid load and osteoporosis, and no protective effect of an alkaline diet on bone.
The cancer claim is thinner still. A systematic review by Fenton and Huang screened more than 8,000 records and found a single study that met the criteria; it concluded there is no basis for promoting an alkaline diet or alkaline water to prevent or treat cancer.
An honest nuance: the diet's food lists (plenty of vegetables, fruit and legumes, less processed meat) overlap with eating that is healthy anyway. The benefit comes from the food, not from pH.
Why the myth exists
The myth runs on an intuition: "acidic" sounds corrosive and harmful, "alkaline" sounds clean and healing. The purity framing feels reasonable without anyone checking the numbers. The pH strips reinforce it. They offer a visible "measurement" that looks scientific, but they read urine, which is built to shift, so you see exactly what you expected. Then there's the money. Alkaline water and ionizer machines, pH drops, supplements and books are a market that needs the premise to be true, and the cancer and osteoporosis angle raises the stakes so fear does the selling. The diet also carries a true kernel (eat more vegetables), which makes the whole package feel validated even though the conclusion does not hold.
What it means for you
Eat more vegetables, fruit and legumes if you like. It's good for you for reasons that have nothing to do with pH. But skip the alkaline water, the pH strips and the ionizer machines; your kidneys and lungs already manage your acid–base balance for free. If you have a real concern, such as kidney stones or a health condition, that belongs with a clinician rather than a food list. Put your attention on total intake and food quality, not on whether it's labelled acidic or alkaline.
