Is BMI actually accurate for telling if I'm healthy?

Only roughly. BMI is weight divided by height squared, and it says nothing about how much of that weight is muscle, fat, or where the fat sits. At the group level it's a useful rough measure, but for an individual it often misses the mark.

What the research says

BMI was built to describe populations, not individual bodies. A meta-analysis of 25 studies covering 31,968 people tested how well BMI captures high body fat measured with proper body-composition methods. At the usual cut-offs, BMI caught only about half of those who actually had elevated body fat (sensitivity 0.50). In other words, it misses half of the people it should flag.

A large US survey (NHANES, 40,420 adults) instead measured metabolic health directly through blood pressure, blood lipids, blood glucose, insulin resistance, and inflammation. There, over 30 percent of "normal weight" people were metabolically unhealthy, while nearly half of "overweight" people and 29 percent of those with obesity were metabolically healthy. BMI category and the measured cardiometabolic markers often pointed in different directions.

The thresholds aren't universal either. A WHO expert review found that people of Asian descent can carry elevated risk of type 2 diabetes and cardiovascular disease even below a BMI of 25, and that sensible risk cut-offs vary between populations.

The takeaway isn't that BMI is useless. It's cheap, fast, and reasonable at the group level. But as a lone verdict on your health it's blunt. Waist measurement, body composition, and blood work tell you more.

Why the myth exists

BMI feels exact because it hands you a number with two decimals and a color-coded category. Precision in form gets mistaken for precision in substance. The measure also survives because it's unbeatably convenient: clinics, apps, and insurers can compute it from two figures everyone already has, with no equipment. What's cheap to measure becomes what gets measured, and then what counts. Who benefits? Whoever needs to sort large groups fast, not necessarily the individual whose health gets summed up in the wrong box. A well-trained person with a lot of muscle can land in "overweight," and a lean person with poor metabolic health in "healthy."

What it means for you

Use BMI as a rough starting point, not a verdict. If your BMI sits near a threshold, that alone tells you little. Waist measurement, how your strength and fitness develop, and blood work if you're unsure all tell you more. If you're building muscle, weight and BMI can rise while your health improves. Let the trend across several measures over time carry more weight than a single number on a single day.
